HOW DO I...
REGISTER TO THE PRACTICE?

You may register as a patient if you live within the practice boundary (and if the practice list is open) by completing a new patient registration form and a health questionnaire. These are available from reception. If you have a chronic health problem such as diabetes, breathing problems, heart disease, epilepsy, blood pressure problems or stroke, please make an appointment to see Janette Sammons, our health care assistant. If you are in good health and are not on any regular medication you will not need to see anyone before making appointments in the usual way. At your first appointment the doctor/nurse will take some basic measurements such as height, weight and blood pressure. All new patients needing repeat medications will need to see the GP/Nurse Clinician.
